Pam Rogers, Chief Executive Officer
Pam Rogers is a seasonal healthcare leader with over a decade of experience in post-acute care settings including skilled nursing facilities, long term acute care hospitals, home health, and inpatient rehabilitation. Holding a Master of Science in Physical Therapy from the University of Colorado Health Sciences Center, Pam brings a clinical prospective and focus on patient centered care.
Throughout her career, she has demonstrated a commitment to driving patient quality outcomes, optimizing efficiency, and fostering strong client relationships.
In her spare time, Pam enjoys spending time with her husband, their triplets and their two labradors retrievers.
Justin Embry, Director of Patient Outcomes
Justin was born and raised in West Lafayette, IN and graduated from Purdue University with a Bachelor’s Degree in Health Science and then went on to receive his Doctorate in Physical Therapy from Chicago Medical School. He has been a physical therapist for over 20 years and is a board certified orthopedic clinical specialist. For 10 of those years, he helped run a balance and vestibular clinic treating inner ear dizziness, concussion, and balance disorders.
Justin has been heavily involved in the fitness industry having owned and coached at a CrossFit gym in Arizona on the border with Mexico. His philosophy is that your health starts with what you put into your body daily and your desire to move it.
Justin loves to hang out with his wife Monica and his college age daughters Gabriela and Sofia. He also has an older son Michael and daughter in law Alissa.
Terri Abbott, BSN, RN, Director of Compliance
Terri has worked in healthcare since 1991 and as a registered nurse since 1996. She earned an associate degree in nursing from Ivy Tech and a bachelor’s degree from Indiana University.
Terri has had the opportunity to work almost every level of care as a nurse from pre-hospital as a flight nurse to long term care as an educator. Terri spent almost 12 years as a contract nurse floating to 24 hospitals in Indiana with experienced in the following areas: ER, ICU, med/surg, telemetry, OB, acute rehab, skilled nursing, care coordination, short stay, PACU, cancer and emotional care centers. Terri feels this vast experience has made the position of Director of Compliance and Risk Management the right fit for her and accepted the position with Ernest in 2021.
In her free-time she enjoys spending time with family and friends. Terri is an accomplished artist and is the Vice President of her local Art Association.
Jeremy Strong, Director of Pharmacy Operations
Jeremy wanted to work in healthcare his whole life, wanting to follow in his mother’s footsteps. Jeremy graduated from Butler University with his Doctor of Pharmacy in May of 2021. He spent the first three years of his career as a clinical pharmacist at Kindred Hospital, an LTACH in Indianapolis, IN.
He is excited to be joining the team at Lafayette Regional Rehabilitation Hospital and continue to use his skills to provide exceptional care to his patients.
In his free time Jeremy enjoys reading, lifting weights, and playing golf.
Ann Miller, Director of Therapy Operations
Ann earned her Bachelor’s degree from Purdue University in 2001 and her Master’s degree in Speech Language Pathology from Ball State University in 2003. She later obtained a Master’s in Business Administration from Ball State University in 2022, specializing in Healthcare Economics. Ann holds a Medical Rehabilitation Management Certificate from AMRPA and is a member of the American College of Healthcare Executives (ACHE). She also has experience as a field surveyor for CARF International (Commission on Accreditation of Rehabilitation Facilities). Ann has dedicated 20 years to serving patients in acute care settings, showcasing her leadership in inpatient acute rehabilitation and acute care therapies. She joined the LRRH team in May 2024, drawn by its team-oriented and patient-focused approach.
In her free time, Ann enjoys spending time with her wonderful husband and six children. She loves to read. Her future goals are to learn to quilt and grow tomatoes.
Trent Riley, Director of Marketing & Business Development
Trent is originally from Lafayette, IN, where he earned a Bachelor’s Degree in Exercise Physiology from Purdue University. He also received an Associate in Science Degree from the University of Indianapolis. Trent has worked as a physical therapist assistant (PTA) in an inpatient rehab setting for over 16 years.
Trent began working at Lafayette Regional Rehabilitation Hospital in 2014 as a PTA and then transitioned to the marketing department in 2018 as a Community Liaison and later as a Clinical Liaison. In 2021, Trent was promoted to the Director of Marketing and Business Development position where he is a motivated advocate for each patient that requires rehabilitation.
Trent enjoys spending time with his family, traveling, and cheering on his children in their sporting activities.
Carolyn Dennison, HR Manager
Carolyn has demonstrated a deep passion for healthcare, a commitment that is rooted in a long-standing family tradition in the field. While she chose to not pursue a clinical path, she discovered her strengths aligned with the relationship developing field of Human Resources. She then pursued her professional education at Purdue University where she later graduated summa cum laude with a bachelor’s degree in Health Care Administration.
In October of 2021, Carolyn began her professional career as a Staffing Coordinator at Lafayette Regional Rehabilitation Hospital, where she excelled and was subsequently promoted to Human Resource Manager. She is enthusiastic about advancing in this role and is dedicated to fostering a positive and supportive environment for employees while maintaining a passionate commitment for their professional development.
Outside of her professional pursuits, Carolyn enjoys spending quality time with her husband and their beloved dog, Sammy.
Matthew Herring, Director of Facilities Management
I am currently working as the Director of Facilities Management at Lafayette Regional Rehabilitation Hospital. I was hired March 22, 2019, as the maintenance mechanic and after 3 years was promoted. Prior to working at LRRH, I worked as an electrician for Skiles Electric out of Frankfort Indiana. For my whole career, I have been involved in fixing things. I worked as a Maintenance Director at ResCare in Sheridan Indiana for 11 years prior to landing at Skiles.
I grew up in Lebanon Indiana where I subsequently graduated high school but was born and raised in the hills of Roanoke Virginia. I enjoy sports, mostly football but my real passion is woodworking in my woodshop at my home. I also enjoy hunting, being outdoors, spending time vacationing with my wife of 25 years and 6 children 12 grandchildren. We have been all over the United States, including Alaska, Hawaii, Wyoming, Florida, the Dakotas, and all over the east coast.
Jimmy Gemino, Food Service Manager
After a decade of experience in restaurants as a cook and manager Jimmy came to Lafayette rehab. Where he earned employee of the year his first year and has been for the past 5 years.
He found a passion for serving a community of people that really impacts change in their wellbeing and seeks gratification in talking with those he serves to further assist them on how to live healthier lifestyles.
Jimmy Studied at the Culinary Art Institute of Indianapolis. He was a culinary Olympics regional winner for Trilogy health services.
When Jimmy is not working, he enjoys coach his kid’s sports, the Colts, Purdue sports, and playing slow pitch softball.
